Prior to booking your medical appointment:

  • Check with your insurance to make sure we are in your network. We are NOT in-network with Medicaid.

  • Please make sure you list the specific reason for your appointment when booking your appointment online.

  • To schedule a medical appointment, click the “BOOK APPOINTMENT” button below.

  • If we need more information you will receive a phone call. Once your appointment has been approved, you will receive a text message and/or email notification.

  • If you do not receive a confirmation notification, please contact our office via telephone to confirm your request.
